Medical Questions to Talk With Your Dr. About

Medical visits, encounters or phone calls can not only be confusing, but tedious as well. Brain injury tends to include the involvement of not just one Dr. or medical professional, but the involvement of many. Life can be depressing if you, you’re caregiver, or advocate are not thorough in obtaining the proper info to permit, encourage & achieve the best results for you to live a positive, abundant, & fabulous life…despite the brain injury effects. The next visit to the Dr./medical professional could be more productive if you ask the following questions or provide the proper info to speed up your appointments. Forms are available through monthly BIA meetings.
* Write your legal name on all forms, provide any changes in address/phone number.
* Write ALL medications (prescriptions, non-prescription, creams, or tonics) you are currently taking…even drugs such as aspirin, antihistamines, vitamins or supplements.
* Write ANY allergies or any allergies you think your Dr. needs to test.
* Write your Med History or any Med History changes since last appointment.
* Write your Primary Care Physician’s name & address, as well as, other Dr’s you are seeing.
* Write Emergency Contact people (include more than 2 or3)--their address & phone number.
* Any Changes in your Appetite? Any Changes in your Mood?
* Any Changes in your body?
* Any changes in Sex? Any changes in Sleep? Any changes in Exercise?
* Any changes in Social Interactions?
* Write down ANY instructions/info from your Dr. or nurse, even if info is given over the phone.
* When medication (pills or creams) are advised, ask the Dr what to do if it is ineffective?
* Does the medication (pills or creams) have any side effects?
* What procedure to follow when medication/treatment creates rash, upset stomach or headache?
* How long should you take the medication or treatment?
* What should you do if you miss a dose of the medication or treatment?
* How long will you need the medication or treatment?
* Will extensive treatment (shots, x-rays, or laser) be needed? Is it covered by your insurance?
* Follow-up on any notes you have taken from your Dr/medical visit or phone call.
